Stroke Club to Learn How PACE North Drives Independent Living

The Grand Traverse Bay Area Stroke Club will host PACE North Community Outreach and Advocacy Director Michelle Reardon at its meeting from 2:30 - 4:30 p.m. on Jan. 8. 

The group meets at The Presbyterian Church, 701 Westminster Road in Traverse City. The public is welcome.

Reardon will detail the opportunities for PACE North to coordinate medical care for stroke patients and others with similar accelerated care needs.

“We’re looking forward to talking with the group members and making sure everyone knows about the PACE program,” Reardon said. “We’re here to help people age in the comfort of their own home.”

The PACE (Program of All-inclusive Care for the Elderly) model enables individuals 55 years and older with state-certified needs for nursing home care to live as independently as possible. Interdisciplinary teams comprised of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides exchange information and solve problems as the health conditions of participants change.

The Grand Traverse Bay Area Stroke Club is open to the public and meets the second Wednesday of each month. The group offers stroke survivors and support persons the opportunity to learn, share, and connect with one another through speakers, demonstrations, and group discussions.
