hour care available in a patient’s home. Home Health services
are tailored and personalized to meet the needs of the individual,
and are met by a multidisciplinary team of health care professionals.
The needs of family are also an emphasis in caring for the patient
and all efforts are made into preserving dignity and independence.
Under the direction of a physician, skilled
professionals provide care on a short-term, intermittent basis
for patients confined to their home due to illness, injury, or
surgery.

Home Telemonitoring
Home telemonitoring is a service we offer at no cost to our patients in an effort to improve service by providing an easy-to-use monitor, which is programmed to meet the patient's needs and convenience. Vital signs, weights, and patient information can be transmitted up to four times a day to a nurse at our central station. The nurse monitors the results and responds as needed, which may include contacting the patient, patient's nurse, and/or patient's physician.
Patients who may benefit most from telemonitoring are those who:
Are
anxious about their condition
Have frequent worsening of their illness
such as CHF
Have frequent hospitalizations
Have frequent visits to the ER
Live in remote areas
Require more teaching
Maternal Child Program
This program provides comprehensive maternal
child services for prenatal, post-partum, and pediatric patients.
Services includes:
High-risk OB patients
Infusion therapy
C-Section complications
Difficulty bonding
Infant malnutrition
Hospital short stay OB program
Lactation education
Infusion
This program is a multi-disciplinary home
service to provide parenteral therapy administered by registered
nurses. Services include:

Maintenance of central venous access devices

Private Services
Private Services provides at-home assistance
on a long-term or temporary basis to fulfill the personal needs
of the patient, as well as household tasks. A free assessment
is provided by a registered nurse to determine specific needs.

Medical Equipment provides respiratory care,
in-home medical equipment, and supplies to rent or purchase. Trained
staff delivers all services and the equipment, is endorsed by
