Here’s what to expect at your dedicated provider visit:

  • A one-on-one appointment with a physician dedicated to managing, educating, and designing a patient plan based on your individual needs and goals
  • During the appointment, you will meet with a medical assistant, a physician, a registered nurse and a registered dietitian. The nurse and dietitian are Certified Diabetes Care and Education Specialists. 
  • Medication management, healthy lifestyle modifications, and recommendations for improved diet and physical activity.
  • A clinic appointment will also cover:
    • Current labs
    • Foot and eye exam recommendations
    • Referral to diabetes self-management education, medical nutrition therapy, and continuous glucose meter education
  • Insulin pump and continuous glucose meter management.
  • Lab services in patient care rooms.
  • Maintain established primary care provider for all other non-diabetes related needs.

Patient Outcomes
