and Benefits
In addition to the unique educational opportunities,
the Munson Family Practice Residency Program offers a comprehensive
benefit package for enrollees.

Benefits Plus Package
Benefits Plus is a "flexible benefits" plan that allows
you to customize your benefits with choices in medical, dental,
vision, life, accidental death and dismemberment, reimbursement
accounts, and dependent coverage. Costs are determined by the
options you select; some elections have no cost while others may
be purchased at special rates.
Medical Care Options
All Munson Medical Center employees, including residents, contribute
to the cost of medical care benefits through payroll deduction.
Currently this amount is set at 17% of the premium cost
Option A Priority Health:
No deductibles; limited co-payments, few out-of-pocket expenses;
all services must originate with HMO providers; includes well-care
and preventative coverage.
Option B Point of Service:
Out of Network services with 20% co-pay. All approved in Network
services same as HMO.
Option C No Medical Coverage:
May select if covered by another source (such as spouse); will
receive bonus waiving coverage.
Coverage effective beginning day one of
employment.
Dental Options
Option A: $800 annual coverage per plan
member; pays 100% for preventative; pays 50% for x-rays, restorative,
and prosthodontics; orthodontia at 50% with lifetime maximum
of $1,000 per dependent child under age 19.
Option B: Covers same services as above,
but with $1,000 annual coverage per plan member and coverage
at 75% for applicable services other than preventative; same
coverage as above for orthodontia.
Coverage effective approximately
90 days after employment.
Optical Discount Plan
Plan available to employee through Eye Care Plan of America (ECPA).
An annual one-time deduction.
Life & Accidental Death Options
Life insurance of one times annual salary is provided. AD&D
coverage is provided up to a maximum of another one times annual
salary. Four options for additional coverage are available, including
one-half times annual
salary, one times annual salary, two times annual salary, and
three times annual salary.
Coverage effective approximately 90 days after employment.
Reimbursement Accounts
Health and dependent care reimbursement accounts allow the use
of pre-tax dollars to pay for qualified expenses, thus providing
tax savings.
Dependent Life Insurance
Employee purchased coverage of $10,000 for each child and spouse
is available, and may be purchased through the convenience of
payroll deductions.
Coverage effective approximately 90 days after employment.
Long-term Disability
Residents are provided with a benefit of 60% of their monthly
wage. This benefit has a six-month waiting period per disability.
Long-term Sick time will be paid out during this time.
Retirement Plans
Munson makes available tax deferred annuity options to residents
wishing to establish accounts through this savings vehicle. Munson
also makes periodic contributions, financial conditions permitting,
to a tax deferred savings
plan.

Holidays
Residents are scheduled on a rotating basis for duties during
regular holidays.
Short/Long-term Time Off
Provides for paid time off for absence due to illness.
Short-term
time off allows for up to three days per year for personal or family illness. These days may be requested by a resident with approval of the Director of Medical Education or the Residency Program Director.
For long-term time off, residents are credited with 1,040 hours
at the beginning of each year to be utilized for extended periods of personal illness.
Accreditation and board certification
requirements necessitate that each resident complete all elements
of the training program. Therefore, time away from the program
due to illness will need to be made up in order to complete the
program requirements. This may require extension of the training program.
Courtesy Allowances
Are provided to Munson employees and legal dependents for pharmaceuticals
and supplies, medical-surgical supplies, and hospital services.
Sick Child/Dependent Care
Provides hospital care during work hours to
permit you to work your regularly scheduled hours.
Other Valuable Benefits
Other benefits provided are: Liability and Travel Insurances,
Employee Assistance Program, Direct Deposit, Leaves of Absence,
Employee Health Program, on-site Credit Union, and Child Care
Center.
Moving Expenses
Up to $500 of moving expenses will be reimbursed to new residents entering the program.
Meals
Meals are provided for residents WHILE ON DUTY only. This courtesy
cannot be extended to families.
License Fee/Membership Dues
First year residents are provided with an educational license
at the hospital's expense. A full license is provided at the hospital's
expense once second and third year residents have met all appropriate
State requirements.
All DO residents are to maintain AOA membership
at all times and will be reimbursed by the
program for the cost of this membership. All DO residents are to maintain membership in the ACOFP (there is no charge for this membership).
All MD residents have the option to join
the AMA or AAFP with the program reimbursing the cost of membership.
If both AAFP and AMA are desired, then the program will pay for
the AMA membership and the AAFP membership can be taken out of
CME funds.
Liability Insurance
Medical liability (malpractice) coverage is provided by MMC for
each resident while performing residency duties. This liability
coverage does not extend to moonlighting, consulting, or other
activities outside of the scope of the residency. It is the resident's
responsibility to arrange liability coverage for all outside employment
activities.
Miscellaneous
ACLS and Neonatal Resuscitation Course
Fees: All residents are required to maintain current certification
in ACLS and Neonatal Resuscitation. Certification and recertification
courses are provided at MMC at no cost to the resident.
Lab Coat Allowance: A $70 lab coat
fee is provided for each year of the residency program. The
Program Coordinator will assist in the order of the lab coats
for the residents.
Pagers: All residents are supplied
with pagers and are required to carry them while on call and
on duty. Residents are responsible for the pagers with the understanding
that if the pager is lost or damaged, replacement will be at
their own expense. If a pager is malfunctioning or not working
accurately, a loaner pager can be given to the resident by the
Program Coordinator at the Family Practice Residency until their
original pager is repaired.
