You can change your health care wishes at any time. You only have
to tell your proxy and physician, and then update any documents
you have. However, it is wise to have a discussion with your physician
prior to making any changes in your documentation.
You may also change your mind about the proxy you
have selected. There are many reasons for this, such as a change
in your relationship with the person, relocation, and concerns
about the person's abilities to support your wishes. You should
inform your proxy of your decision and then complete revised documents
as necessary to reflect the change. If you make any changes in
your documents, be certain to replace old documents with the revised
version. We recommend you keep a list of the location of copies of your advance directive.
