Providing primary care in a small community allows us to get to know our patients and their families well—we enjoy seeing both adults and children in our office. From well child check-ups and minor injuries to acute illness and chronic health conditions, we’ll take great care of you and your family. We Are a Patient-Centered Medical Home We’re proud to be designated Blue Cross Blue Shield of Michigan Patient-Centered Medical Home practice since 2010. We have worked very hard to achieve and maintain our PCMH designation. We’re committed to placing the focus on you, our patient. Instead of just treating you for various problems, our Patient-Centered Medical Home works to coordinate your care and keep you as healthy as possible. Our team of healthcare professionals is dedicated to improving your overall health and to helping you reach your health goals. This team may consist of a primary care doctor, nurses, specialty physicians, pharmacists, diabetic educator, care coordinators, and others, depending on your needs. We’ll be sure to follow up with you to see how you’re doing or remind you to schedule any necessary tests to help us manage your condition. You’ll also have 24-hour access to someone in our practice. That means that if you have a serious health problem in the middle of night or on a weekend, your Patient-Centered Medical Home team is available — which could help you avoid a trip to the emergency room. If you are looking for a more personal approach to healthcare for you and your family, our “medical home” will care for your family as we would care for ourselves.
