The following documents are required to be completed before the start of your clinical rotation:

Student Orientation/Safety Education

To be completed by all students (**NMC students will complete steps 3 - 5 through NMC's online learning system):

  5. Complete test and give to your college instructor/ representative

HIPAA Privacy and Security Course: Initial

To be completed by new students (**NMC students will complete steps 3 - 5 through NMC's online learning system):

  4. Print the test
  5. Complete test and give to your college instructor/ representative

HIPAA Privacy and Security Course: Annual Renewal

For students who have completed the initial course and have returned for a second or third year (**NMC students will complete this process through NMC's online learning system):

  3. Complete the test and give to college instructor/representative
  4. Note:  If you did not pass the test, you must repeat the initial course.  See above for instructions

Confidentiality & Systems Usage Agreement

To be completed by all students:

  3. Print the forms
  4. Read and sign the forms
  5. Give the signed forms to your college instructor/representative

Parking Registration

Please note that NMC and Baker nursing students in clinical rotations and TBA Career Tech Center students have a different process to obtain a parking permit and not required to complete this form.

Instructions for all other students who are not currently MMC employees:

  3. Complete the form and bring to MMC Human Resources on your first day on-site to receive a temporary parking permit.

ALL STUDENTS must park in Lot Q (including MHC employees who are at MMC in a student capacity) and display a valid parking permit.

Computer System Access Request Form (Student Nurses ONLY)

Instructions:
