To apply by mail, download the application for health coverage from the State of Michigan website and return the completed form to the address noted. Visit michigan.gov/medicaid for more information. Copies of the form can also be picked up at Munson Healthcare [locations].

Medicaid Eligibility Renewals

Important Steps

  1. Update your address, phone number, and email address at michigan.gov/mibridges or contact your local MDHHS office.
  2. When you receive your renewal packet, report any changes to your household or income. 
  3. Return your completed packet to your local MDHHS office in person or by mail or submit it online. Be sure to submit by the due date or your coverage could lapse. 

If you are no longer eligible for Medicaid coverage, you can purchase health insurance through HealthCare.gov.
