Information for Charlevoix Hospital Patients and Visitors 

Patients and their families are our top concern at Charlevoix Hospital. We want to make sure your time with us goes as smoothly as possible. Whether you're here for an appointment, a procedure, or you're a visitor, we want you to have a good experience.

We'll Take Good Care of You

We will do our best to make your stay as comfortable as possible and start you on the road to recovery.

You will meet many caregivers while you are here – physicians, nurses, technicians, and other health professionals. All of us share one common goal: to help you recover your health.

Registration and Admitting

You will be welcomed by registration and admitting staff, located just inside the hospital’s main entrance to the left of the Information Desk. We will ask you a few questions about your health history and complete the necessary paperwork. (If you are coming in for lab work and have registered with us in the past year, you may go directly to the Lab; no need to stop at Registration.)

What to Bring

  • Your health insurance card

  • Your social security number

  • A list of all medications you take (prescriptions and over-the-counter), along with the dosage

Items to Bring if You are Staying Overnight

  • Robe, slippers, pajamas

  • Personal care items and toiletries

  • A book or magazine

  • List of family phone numbers in case you want to make phone calls

  • Containers for eyeglasses, dentures, hearing aids, or other personal property

Please leave your jewelry and other valuables at home or send them home with family. When that’s not possible, you may deposit money and other valuable items in the hospital safe while you are here.

Your Health Care Team

You will be cared for by a team of professionals who will:

  • Provide highly-skilled, quality care

  • Treat you with respect, courtesy, and compassion

  • Listen carefully to your needs

  • Provide clear explanations about your care and medications

  • Effectively manage your pain

  • Provide a clean, comfortable, and quiet environment

  • Help you plan for your care after you leave the hospital

If You Have a Question, Concern, Compliment

We expect your stay to be problem-free. However, we welcome you to contact the nurse manager of the department providing your care if you have:

  • Questions about your care or your stay

  • Concerns or comments related to your care

  • Compliments and recognition for staff delivering your care

After You Leave

Your care does not end when you leave our facilities. We are always here to answer any questions once you are recovering in the comfort of your home. You may wish to request continued care at home, have questions about your bill, or want to thank a caregiver and share your positive experience. Please feel free to give us a call at 231-547-4024.
