Care Management at Manistee Hospital

At Manistee Hospital, we care about you as a whole person, and not just about your illness. Managing your health care can be confusing, especially if you have a chronic illness. As part of your patient-centered medical home, Manistee Hospital’s care management services can help you live the best life possible.

What is Care Management?

Care management is for people with multiple chronic conditions, including diabetes, asthma, chronic obstructive pulmonary disease (COPD), and heart disease. Care management is patient-centered and completely voluntary. If you qualify and choose to participate, you will be asked to take an active role in your health, helping to develop and follow a plan of care. Care management is covered by some insurances.

Your Care Coordinator

A care coordinator will work with you one-on-one to help you better understand your conditions and develop an individualized plan to help you manage your health. The coordinator will be in regular contact to see how you are doing.
