Medical Case Management

Medical case managers are passionate about their work and can assist with:

  • Care coordination between primary care provider and other specialists, including infectious disease doctors, dentists, hearing services, vision care

  • Cost of care

  • Employment

  • Fear of losing anonymity

  • Finding community resources

  • Lack of health insurance

  • Referrals to housing resources

  • Referrals to specialists (cardiology, dermatology, gastroenterology, etc.)

  • Transportation needs

Home Visits Via Telemedicine

Established clients may be eligible for home visits via telemedicine. For those unable to travel comfortably, the Thomas Judd Care Center can schedule follow-up visits via a travelling nurse and state-of-the-art telemedicine equipment. Please discuss this option with your case manager.

Pharmacy Support*

Access to a pharmacist who specializes in HIV/AIDS to answer questions about drug interactions and medication side effects.

Nutritional Support*

A registered dietitian addresses weight management and resources to help cover cost of vitamins and nutritional support.

Mental Health and Substance Abuse Counseling

One-on-one and psychosocial support during events for those living with HIV/AIDS, their partners, and family members.

 

*There are caps on financial support offered by Thomas Judd Care Center, designated a “payer of last resort” after all other options are exhausted. A household budget/financial analysis worksheet must be completed by each client.
