Collaborative Practice and Prescriptive Authority Delegation Agreements

Munson Healthcare has completed a review and update of APP collaborative practice and prescriptive authority agreements to meet the Michigan and Federal guidelines.

What you need to know: 

  • Practice managers will now be responsible for ensuring that practice agreements are completed, on file in the office, and updated as needed.
  • The appropriate practice agreement should be completed for all Nurse Practitioners, Nurse Midwives, CRNAs, or Physician Assistants. A Prescriptive Authority should be maintained for any Nurse Practitioner or Nurse Midwife prescribing controlled substances.
  • Per federal regulation and Michigan law, the APP and supervising physician are required to maintain a copy of these delegations at the primary place of practice.
  • APPs and/or their practices must be able to provide this agreement to MDHHS, surveyors, hospital departments, or other institutions immediately upon request.
  • Prescriptive authorities should be reviewed annually.
  • Practice agreements should be updated as needed if supervising arrangements change.
  • The Medical Staff Office at each facility should be informed of any changes in an APPs supervising physician.
  • The updated MHC forms can be found on the For Providers website at: (include link).  
  • Non-employed practices may utilize these forms, or their own, for APPs credentialed at MHC facilities. They are still expected to provide copies when requested, to MDHHS, surveyors, hospital departments, or other institutions immediately upon request.
