Referring a Patient to an MHC Emergency Department

Information You’ll Be Asked to Provide

Please provide as much clinical detail as possible – see list below. If a callback is needed, include the appropriate contact number.

  • Caller name (person initiating the pre-arrival call)
  • Sending facility name (clinic, urgent care, SNF, etc.)
  • Patient’s full name (first and last)
  • Patient’s age and/or date of birth
  • Chief complaint
  • Current vital signs
  • Mode of arrival (ambulance, private vehicle, walking, etc.)
  • Current medical complaint/relevant medical history/working diagnosis 
  • Brief description of the patient’s condition
  • Pertinent clinical events or changes
  • Sending provider’s expectations (patient needs radiology imaging, labs, evaluation for hospital admission, etc.)
