Preparing for Your Appointment
We see new patients by referral from physician only. If you are a new patient, we ask that you arrive 20 minutes before your scheduled appointment time.
All patients should bring their insurance card to each visit.
We understand that your schedule may change and you may need to cancel an appointment. If you need to cancel, please contact us at least 24 hours prior to your scheduled appointment.
Pay Your Bill
Services must be paid for in full on the day received, unless other arrangements have been made in advance. We work closely with multiple insurance companies and will bill them on your behalf. However, all co-pays, deductibles and co-insurance amounts must be paid on the date of service. If you have any questions about your insurance coverage or billing statement, please call 800-637-4033.
To pay your bill:
- By Mail: Send payments to the address listed on your bill.
- By Phone: Call our Central Billing Office at 231-935-6160.
- In Person: Payments can be made at our office at 1200 Sixth St., Ste. 200, Traverse City, Michigan.
Prescriptions may be renewed through our patient portal. Patients may also contact our office at 800-637-4033 during office hours. In some instances, we may require an office visit prior to refilling a prescription.
