Individualized Care Plans

Since no two patients are exactly alike, we tailor cognitive rehabilitation programs to each person’s specific needs.

Evaluations, both standardized assessments and several informal assessments, help us first determine each patient’s strengths and weaknesses. Once the evaluations are complete, our team meets to design an individualized care program to include therapist identified areas of need as well as patient generated goals.

Personal progress is carefully monitored and celebrated, too. Throughout the course of rehabilitation, we celebrate every success, large or small, on the path to improved cognitive, physical, and emotional well-being.

Family and Caregiver Involvement

We know recovery is faster and more successful when family members and caregivers are involved in treatment.

That’s why our team holds monthly meetings for each patient, inviting family members and caregivers to join us to learn our techniques and strategies. In addition to these meetings, family members and caregivers are encouraged to sit in on individual sessions, and also participate in treatment tasks as their schedules allow. Understanding therapies and using the same ones at home can reinforce the care their loved ones are receiving.
