Munson Healthcare’s monoclonal antibody (mAb) therapy program continues to experience significant resource constraints and an enormous amount of referrals coming in from our medical community and from our self-referral program. In response to provider feedback and to help create efficiencies in the referral process, we have launched a new, secure web-based patient referral form. Only providers are permitted to use this form, patients will need to call the Ask-A-Nurse line to self-refer. This new web-based form will replace the current MHC mAb Pre-Screening and Referral Checklist.
Beginning Wednesday, October 27, use the secure mAb web-based referral form to refer your patient for monoclonal antibody (mAb) therapy. This form is also available on our For Providers website.
- To remain HIPAA compliant, your initial referral will require two-factor authentication using your Munson Healthcare network login (i.e., Citrix StoreFront login) if you are accessing the form from outside our network; any subsequent referrals will not.
- After Friday, November 5, referrals to the mAB clinic will only be processed using the web-based form; faxes, old forms submitted from EMR, RighFax, and/or paper forms will no longer be available or processed.
The current volume of patients who meet the criteria for monoclonal antibody (mAb) therapy exceeds the number of appointments available so not all patients you refer for treatment will be able to be treated. We are working on processes to notify providers and patients when we are unable to schedule them for treatment.
