Providers Asked to Report Post COVID-19 Vaccination Breakthroughs and Unusual Complications

The state is asking that providers report the following:

  • Breakthrough cases of fully immunized individuals who test positive for COVID-19.
    • A breakthrough case is a person who:

      • has completed an approved vaccine series

      • is 14 days past their last dose (regardless of manufacturer)

      • has COVID proven by either PCR or antigen test, and

      • has not had a positive COVID test in the proceeding for 45 days

    • NOTE: As of April 5, there have only been 246 breakthrough cases in Michigan, which is a very small percentage of the more than 2 million individuals who have been vaccinated to date so the vaccines are very effective.
  • Suspect an unusual complication after COVID-19 vaccination (more than sore arm or low grade fever for a day or two).

The state uses this data to determine patterns/clusters (e.g., bad batch of vaccine due to temperature instability, does patient have immune deficiency, individuals with a common health history, related to variants, etc.) and the safety/efficacy of the vaccines.

We are capturing this data if the patient comes into a Munson Healthcare facility, but we ask primary care providers to help us track these patients. If a COVID-19 vaccinated patient comes into your office who is either a breakthrough case or has unusual post-vaccination complication, please use VOICE to report, which is then tracked and entered into VAERS (Vaccine Adverse Event Reporting System), if applicable.

VOICE is located on the Munson Healthcare Intranet.* When submitting a VOICE report:

  1. Select Medication/Fluid
  2. Specific Event Type: Adverse Drug Reaction
  3. Description: Please note whether your office has already submitted to the state or VAERS

If you are unable to access the Intranet, please contact Patient Safety at your hospital to submit a VOICE report.

 

*NOTE: Charlevoix Hospital and Manistee Hospital have different methods for accessing VOICE
