Community Health Needs Assessment - Charlevoix Hospital
What is it?
Every three years, the Affordable Care Act requires nonprofit hospitals to conduct a Community Health Needs Assessment (CHNA). Our 2015 assessment represents a multi-community project, undertaken to identify, evaluate, and prioritize the health status and needs facing our community. The assessment serves as the basis for all participants of our health community to work together to design, develop, and implement programs to effectively address identified needs.
The 2015 CHNA reports on current health status, health factors and barriers to care, and other demographic and social issues affecting the health of people and organizations in our eight-county area. The assessment covers Alpena, Antrim, Charlevoix, Cheboygan, Emmet, Montmorency, Otsego, and Presque Isle counties, with a focus on Antrim and Charlevoix counties. These areas are primarily serviced by Munson Healthcare Charlevoix Hospital, McLaren Northern Michigan, Munson Medical Center, and Otsego Memorial Hospital.
Munson Healthcare Charlevoix Hospital is committed to improving the health of the communities we serve. In response to the 2015 Community Health Needs Assessment, we will develop an implementation plan, describing existing programs to address identified health needs and outlining program expansions and new programs we plan to implement to address those needs. Together, we can make our community a healthier place to live, work, and visit.
Printed versions of both the 2015 Community Health Needs Assessment and Munson Healthcare Charlevoix Hospital’s current Health Improvement Action Plan are available for review at Charlevoix Public Library, Boyne District Library, and Jordan Valley District Library.
