Financial Assistance Programs
Otsego Memorial Hospital may be able to help you with payment options and financial assistance if you have difficulty paying your medical bills.
Our patient financial advocates in Patient Financial Services can:
- Estimate what your portion of a bill will be.
- Determine if you qualify for financial assistance or reduced prices.
- Help you make appropriate payment arrangements.
If you think you may be eligible for assistance, use these links to print, review, and complete the “Assistance Application & Program Summary”, which includes a copy of the Financial Assistance “Plain Language Summary” description of program requirements.
Bring the completed form and any applicable additional documentation to Patient Financial Services located in the OMH Administrative Services Building, 271 West McCoy Rd., Gaylord MI, or mail to 825 N. Center Ave., Gaylord MI 49735. The office is open Monday through Friday, 8 am to 4 pm.
